Welcome to Rise Park Surgery

Rise Park Surgery serves the whole of Rise Park as well as areas of Top Valley and Bestwood Village.

Our team includes seven GPs, three nurses, one senior health care assistant, three phlebotomists as well as our Business Manager, Patient Services Manager, administration and reception staff. We offer a full general practice service and run specialist clinics for children and pregnant women, diabetes and asthma sufferers and for patients needing minor surgery.

Just to let you know …

 

The Surgery will be closed on Tuesday 17th November 2020 from 12.30pm for training purposes.

We will re-open at 8am on Wednesday 18th November 2020.

 

Any calls to the surgery will be automatically diverted to the NEMS Out of Hours Service or alternatively please call 111 if you need urgent medical attention.

Rise Park Surgery Flu Clinics Autumn/Winter 2020

 

We are pleased to announce the dates for our 2020 flu campaign.  Due to the restrictions placed upon us during the COVID pandemic we will taking precautions to protect both you our patients and our colleagues and so the clinics will operate differently this year.  Please take a few minutes to read the information below.  The clinics will be held on the following days:-

Saturday 10 October 9am-12noon

THIS CLINIC WILL **ONLY** BE FOR PATIENTS AGED 65 YEARS AND OVER.

Patients whose family name/surname starts with letter A-M will be able to attend between 9am-10.30am

Patients whose family name / surname starts with letter N- Z should attend between 10.30am-12noon.

Saturday 17 October 9am – 12noon

THIS CLINIC WILL BE FOR ANY ELIGIBLE PATIENT OVER THE AGE OF 18 YEARS

Patients whose family name/surname starts with letter A-M will be able to attend between 9am-10.30am

Patients whose family name / surname starts with letter N- Z should attend between 10.30am-12noon.

Please note:-

  • Flu clinics for eligible patients under the age of 18 will be held on an appointment basis; we will contact patient/parents/carers as and when the vaccine becomes available.  

 

  • If you are unable to attend any of the clinics please call the Surgery (after 11am) to arrange an appointment with Cheryl, Jane or Samantha.

 

  • Please note that wherever possible only the person having the vaccination should attend the clinic; carers are obviously exempt from this advice.

 

  • These dates are subject to change due to government guidance and vaccine delivery schedules.

 

  • You must wear a face covering to protect yourself, other patients and staff.

 

  • Please be patient and be aware that you may have to queue for a short time outside or in your car.

 

  • When you enter the building please make your arm ready and remember to wear loose or easy to remove clothing.

 

Appointments at Rise Park Surgery

We are continuing to offer telephone consultations to our patients.  If the GP or Nurse needs to see you they may ask you to have a video consultation with them or they will invite you to a face to face appointment at the surgery if that is more appropriate.  Please do not attend the surgery if you have symptoms of COVID-19.

* Please wear a face covering to appointments to protect yourself and others
* Where you can please attend appointments alone and not in groups
* Please wash you hands before and after your appointment
* Please wait in your car or outside the building until your appointment time and then ring the bell at the front door to let us know that you have arrived.
* Please remember social distancing guidelines
These measures are in place for your safety and all that work at the surgery.

Face mask exemption 

Whilst we appreciate that these are challenging times for our patients, is it not the responsibility of a GP to issue an exemption letter to an individual from not wearing a face mask within various public settings.

COVID-19/Corona Virus Isolation Note

If you are isolating for whatever reason (i.e. you feel unwell yourself or someone in your household is unwell), and you need a note for your employer please be advised that we are not issuing ‘fit notes’ relating to COVID-19/ Corona Virus.

Please see the link below for a letter signed by Dr Mark Salisbury. You can print this off at home and this should be sufficient for your employer.

If you do not have a printer at home please contact us and we will try and help.

COVID-19/Corona Virus

Apologies for any inconvenience but the GP Partners and Managers have taken the difficult decision to stop face to face consultations from Friday 13 March 2020.  This is due to the high risk of infection from COVID 19/Corona Virus.

If you have an appointment booked already you will be added to the GP/Nurse Prescriber triage list and brought down to see us as needed.  This means you may still be seen by a Doctor or Nurse but everyone will need to be screened prior to attending the surgery.

If you need a repeat prescription, please submit a request via the website (you need to have previously registered for this service) or telephone reception and we take your request over the telephone (this is a temporary measure).  Please be prepared to nominate a pharmacy as we will send your request directly to them.

Please bear with us whilst this is happening and once again and we apologise for any inconvenience this may cause; we have taken this action to keep everyone safe.

Wherever possible we try to use data that does not identify you, but sometimes it is necessary to use your confidential patient information.

 

What is confidential patient information?

Confidential patient information identifies you and says something about your health, care or treatment. You would expect this information to be kept private. Information that only identifies you, like your name and address, is not considered confidential patient information and still may be used: for example, to contact you if your GP practice is merging with another.

 

Who can use your confidential patient information for research and planning?

Is used by the NHS, local authorities, university and hospital researchers, medical colleges and pharmaceutical researching new treatments.

 

Making your data opt-out choice

You can choose to opt out of sharing your confidential patient information for research and planning. There may still be times when confidential patient information is used: for example, during an epidemic where there may be a risk to you or to other people’s health. You can also still consent to take part in specific research project.

 

Will choosing this opt-out affect your care and treatment?

No, your confidential patient information will still be used for your individual  care. Choosing to opt out will not affect your care or treatment. You will still be invited for screening Services, such as screenings for bowel cancer.

 

What should you do next?

You do not need to do anything if you are happy about how your confidential patient information is used.

If you do not want your confidential patient information to be used for research and planning, you can choose to opt out securely online or through a telephone service.

Rise Park Surgery Extension to the Existing Surgery Building

The GP Partners and staff are pleased to announce that an application has recently been submitted to the relevant planning authority for an extension to the existing surgery building.

If the plans are passed as they are the clinical space at the Surgery will nearly double in size meaning that we are better placed to care for the community now and in the future.

Once the plans are passed we will have a display available in Reception.


Phlebotomy/Blood Taking Sessions

Just to let you know that occasionally there may be a need to have 2 phlebotomists working out of the same room due to capacity issues here at the Surgery.  This may mean that you could be having your blood taken at the same time as someone else and also someone of a different gender to yourself.  We appreciate that this isn't ideal for all patients so if you would rather re-book for another time please let the phlebotomist or receptionist know.

We have taken the decision to operate the clinic with 2 phlebotomists rather than one to try and keep waiting times down to a minimum and we very much appreciate your co-operation and patience at this time.


Just to let you know...

During these unprecedented times, the surgery can and will take prescription requests over the telephone.

Please do not come to the surgery unnecessarily as you will need to call ahead for your query. Our receptionists will guide you in a safe manner.

Alternatively, you can sign up for our online services and request your prescription electronically, your prescription will be processed and sent to a pharmacy of your choice. Speak to a receptionist to find out more.

GP+ Nottingham City - Extended Hours Access for Patients

 

 GP+ offers evening & weekend appointments bookable through your GP practice, patients registered at this practice are able to access additional routine appointments during evenings and weekends through the new GP+ service.

 

Appointments are available to see GPs, Practice Nurses, Clinical Pharmacists and Physiotherapists in a fully equipped accessible location on Upper Parliament Street in Nottingham City Centre.

 

Opening hours are:

 

16:00-20:00 Monday – Friday

9:00-13:00 Saturday & Sunday

Rise Park Surgery are pleased to announce that we are able to offer patients the ability to order repeat prescriptions via this website as well as booking appointments on line and view your medical record.

To register for the website please bring one form of photo ID (i.e. passport, driving license) and one utility bill showing your current address (no more than 6 months old) and reception will provide you with a user name and password.
